About the role

The Coordinator – Client Relationships is responsible for client engagement and entry into Northside Aged Care and Inclusion programs with a particular focus on the services delivered under the Commonwealth Home Support (CHSP), the Home Care Package (HCP) and Veterans' Home Care (VHC) programs.

The position works closely with the Operations & Social Inclusion teams with the aim of ensuring all individuals accessing the programs have a seamless, respectful and positive experience from initial enquiry through to the commencement of services, monitoring of delivery and client feedback.

We are looking for an exceptional person who has extensive experience working in client centred model of practice. This person will be able to work collaboratively, have a strong understanding of sector reforms and demonstrated knowledge of the needs and issues faced by individuals who are ageing/or live in complex situations. This person will be the key point of contact for all initial enquiries, new referrals and as such will develop strong working relationships with referral agencies and other community organisation teams.
