Your Opportunity

HOPARRS – Homelessness outreach prevention and rapid rehousing service is a Specialist Homelessness Service that provides support through client centred case management, assertive outreach, referral and advocacy to Single men, women and families in Blacktown and Parramatta LGA’s

The role of Case Worker is to support clients experiencing homelessness or at risk of homelessness to establish or sustain tenancies.

Position Responsibilities

  • Respond to referrals of clients to the service from internal and external support services and conduct over-the-phone and formal face-to-face assessments of suitability for support
  • Undertake initial registrations for clients, including all necessary paperwork.
  • Entering client information into Mission Australia’s client management system
  • Work with clients to create individualised case plans including referral to supplementary services as needed
  • Provide intensive ongoing case management sessions (formal and informal) with clients, conduct regular reviews and provide additional supports as required.
  • Develop strong relationships with key external stakeholders including other service providers, community service workers, government agencies
  • Provide outreach services at key first to know agencies, e.g. Housing New South Wales, Centrelink and homelessness hubs.
  • Oversight of a men’s transitional accommodation property

Qualifications & experience

  • The successful candidate must have knowledge and/or experience in working with clients in a residential setting
  • A Degree or Diploma in Social Welfare/Social Science or the related humanities
  • Experience and or knowledge of the homelessness sector
  • 2 or more years’ experience in case management
  • A current NSW Driver’s License
  • Senior First Aid Certificate
