Your new role

We are seeking to appoint an experienced Manager to drive strategic projects investigating, exploring and establishing development opportunities across a breadth of aged and primary care areas at Merri Health. Reporting directly to the General Manager, Aged & Primary Care you will be responsible for:

  • Coordinating responses to industry reforms, initiatives and growth opportunities
  • Redesign and repositioning of services to achieve better integration and service coordination in a competitive market;
  • Innovative models of transdisciplinary care to support workforce innovation and new models of care; and
  • Working to strengthen the interface between acute and primary care.
  • Stakeholder management of key partnerships and relationships
  • Co-design with consumers
  • Promoting an integrated and client centered approach to care

Based at our Merri Central site located in Coburg, this is a full time position offered on an ongoing basis.

What you'll need to succeed

To be successful with Merri you will go above and beyond to achieve great results for our clients, the organisation and yourself. We're looking for an individual who:

  • Has tertiary qualifications in public health, aged care, allied health or nursing
  • Has proven project management experience in multidisciplinary aged care, chronic care, allied health and/or allied health field
  • Has experience in research and evaluation
  • Is KPI driven and target focused
  • Is keen to drive change and innovation
  • Is a problem solver and a fast thinker
  • Is committed to delivering quality services with a client centered approach
