About the role:

This role works to actively support the individual needs of frail, aged and people with a disability who access Life Without Barriers Home and Community care Services. HACC Support Workers work as a team to facilitate the delivery of quality services and achieve positive outcomes with clients in a positive and proactive environment.

Key responsibilities:

  • Provide quality direct support to clients in line with their support needs, the program they are accessing and all support plans including their Care Plan
  • Increase the client’s skills, independence personal growth and development through provision of active support
  • Keep accurate and timely records and ensure that appropriate records are updated and maintained
  • Assist the client with showering, bathing, eating and any personal care needs
  • Undertake domestic duties such as cleaning, washing cooking
  • Assist client with community access as identified on the clients care plan

Skills & experience:

  • Certificate III in Aged Care or Certificate III in Individual Support (Ageing)
  • Current unrestricted drivers licence
  • Current First Aid Certificate or willingness to obtain
  • Current, or willingness to undertake, Food Handlers Certificate
  • A commitment to supporting and promoting the individual needs of clients
  • Experience in providing personal care or domestic support and assistance to clients
  • Good communication skills

Successful candidates will be required to clear probity checks including National Criminal History Record Check and Working with Children Check (where relevant to the role).
