About the role:

As a Support Worker, you will actively support the individual needs of people who access Life Without Barriers' disability and out of home care services. You will be responsible for supporting and promoting their skills and abilities, providing support with activities of daily living and developing, implementing and monitoring their Support Plan. 

Please note: Our core business is providing 24/7 support to clients in their own homes. Therefore, the successful applicants for this Permanent Part Time role will be required to work across a range of shifts including sleepovers, occasional weekends, and public holidays.

Key responsibilities:

  • Assistance with daily living (including personal care)
  • Community access and transport
  • Maintain and monitor person-centred plans
  • Documenting and progress note keeping
  • Following and adhering to Positive Behaviour Support plans and restrictive practices where applicable

Skills & experience:

  • Unrestricted Australian Drivers Licence - Essential
  • Certificate III Individual Support, Cert III in Aged Care, Community Services or higher - Essential
  • Current Provide First Aid Certificate (or willingness to obtain)
  • Excellent Customer Service skills
  • Experience working with Complex Behaviours
  • Passionate about delivering Person-Centred care

Critically, your experience supporting high needs customers with Positive Behaviour Support Plans and passion for providing Active Support will drive you to make a real difference to the lives of those we support.
Successful candidates will be required to clear probity checks including National Criminal History Record Check.
