About the Role

You will provide practical assistance to people with mental illness to help them achieve their personal goals, develop better relationships with family and friends, and manage their everyday tasks. Clients are assisted to access services and participate economically and socially in the community, increasing their opportunities for recovery.

You will be required to deliver coordinated, integrated and complementary support to participants living with mental health issues while aiding in their recovery journey, assisting them to building relationships and providing holistic support.

We have a 10 hours per week, 12 month part-time contract available working with a client’s North and South Metro, you must be willing to travel across the Perth metro area. There will be an opportunity to take on additional shifts as they become available.

Shift days/times will depend on the support plans but generally most support is required during the day Monday-Sunday. 

Key Responsibilities

  • Support consumers to work towards achieving their individual aims
  • Support consumers to build their capacity
  • Support consumers to test eligibility and transition to the NDIS
  • Liaise and collaborate with consumers, their community and community support agencies
  • Monitor and review client’s progress through the service and against the case plan
  • Seek to understand client’s expectations
  • Sensitively ascertain information about the consumer’s health, family and cultural background
  • Coordinate the delivery of services to consumers and work with the client to utilise other referral agencies and services

Skills & Experience

  • Minimum Certificate IV in Mental Health
  • Current Senior First Aid Certificate
  • Current Driver Licence and vehicle 
  • Well-developed verbal and written communication skills
  • Positively engage, interact and work with people who have complex needs including a psycho-social disability
  • Possess a comprehensive knowledge of the issues faced by people with mental health issues

Successful candidates will be required to clear probity checks including National Criminal History Record Check and Working with Children Check.
