The Role

The Stepped Care program is a collaboration between Connect4Health consortium partners (AccessHC, Carrington Health and Link Health & Community), with AccessHC as the lead agency.

There are 3 Permanent Part time positions available for the Mental Health Nurse role and the incumbents will be located at AccessHC (Box Hill and Glen Waverley sites) and provide an integrated and accessible clinical mental health nursing service, including coordination of clinical care to consumers with severe mental illness and complex needs.

Clients may present with a range of episodic mental health conditions, including mood disorders, trauma, anxiety disorders and psychotic disorders. The aim of the role is to support clients to remain in the community, and to liaise closely with the client’s care team which may include a GP, psychiatrist, or other stepped care services.

Skills and Experience

  • Division 1 Nurse with current registration with AHPRA
  • Qualification in Mental Health Nursing
  • Credentialed Mental Health Nurse with Australian College of Mental Health Nurses
