Position Overview

The Getting it Together Case Worker will work with young persons to address need using a strengths based approach. The Case Worker will work collaboratively with the client to develop a case plan; coordinate and plan a mix of services to meet the young person’s needs and monitor and evaluate the effectiveness of the intervention provided. Brokerage can be used to purchase services and/or items for clients where needed and as supported by demonstrated priority need in the case plan. The Getting it Together Case Worker will also develop, coordinate and/or implement a range of group interventions relevant to the needs of the young people accessing services.

Position Responsibilities

The Case Worker will work in a collaborative team with another staff member and will assume overall accountability and responsibility for delivering a professional service that aims to meet benchmarks set for the program.

The Getting it Together program aims to reduce alcohol and other drug use by liaising with the community and supporting young people to bring positive change in their lives by:

  1. Creating ongoing positive family and community relationships
  2. Building self-esteem, personal growth and life skills
  3. Addressing personal issues and health problems
  4. Understanding of the impacts of alcohol and other drugs
  5. Promoting harm minimisation and addressing alcohol and other drugs misuse
  6. Connecting to specialist services where appropriate

The Case Worker will achieve these aims by:

  1. Using assertive outreach in delivering services
  2. Building and developing effective partnerships and networks
  3. Providing brief interventions and time limited support
  4. Providing case management using a biopsychosocial approach
  5. Working within a harm minimisation framework
  6. Developing, implementing and evaluating effective group interventions
  7. Demonstrating best practice guided by the policies and procedures of the organisation

Please note that this is a 3-6 month contract to cover maternity leave.
