The Getting it Together Case Worker will work with young persons to address need using a strengths based approach. The Case Worker will work collaboratively with the client to develop a case plan; coordinate and plan a mix of services to meet the young person’s needs and monitor and evaluate the effectiveness of the intervention provided. Brokerage can be used to purchase services and/or items for clients where needed and as supported by demonstrated priority need in the case plan. The Getting it Together Case Worker will also develop, coordinate and/or implement a range of group interventions relevant to the needs of the young people accessing services.
The Case Worker will work in a collaborative team with another staff member and will assume overall accountability and responsibility for delivering a professional service that aims to meet benchmarks set for the program.
