Your Community Health is seeking an enthusiastic and experienced Social Support Program (SSP) Worker to join the Social Support Program team in this part-time role, working 4.5 hours per week. This is a fixed term position commencing mid October 2019 until the end of June 2020.

Your Community Health provides a comprehensive range of medical, dental and allied health services and support programs to anyone, through a combination of outreach, client-based, and centre-based activities. Our purpose is to enable health, wellbeing, and dignity for all people in northern Melbourne by providing responsive and accessible services. We provide priority access to those most at risk of health inequalities and those that may face barriers to accessing traditional health services.

About the role

Reporting to the Social Support Program Manager, the SSP Worker will assist with the day to day implementation of the SSP Program. The aim of the Social Support Program (SSP) is to provide high quality and meaningful activities for frail aged and disabled people, in a comfortable environment, which promotes client independence and participation.

Your passion and commitment will see you:

  • Facilitate group activities for people with Multiple sclerosis (MS)
  • Assist in planning and delivery of physically and intellectually stimulating programs and activities for participants
  • Work co-operatively as part of the SSP team under the guidance and direction of the SSP Manager within the policies and procedures of the organisation
  • Have a working knowledge of all emergency procedures and ensuring client safety at all times
  • Provide personal care for clients as required

To succeed in this role you will have:

  • Qualifications minimum Certificate III in Individual Support (Aged Care) or Certificate IV in Lifestyle and Leisure, Disability, or be working towards gaining qualifications
  • Experience and enjoyment of working in the aged care, disability and community services area, with an appreciation of the needs of the client group
  • Ability to work within a team
  • Skills in managing time, setting goals and planning work priorities to meet goals and objectives
  • A second language is desirable
