About the role

The Community Registered Nurse, Mental Health position will primarily focus on working closely with General Practices (GPs) and their consumers in the Metro region of Perth. This role involves providing brief interventions, linking consumers with community service organisations, offering a stepped care service, and responding to complex needs within the community.

You will be working alongside consumers experiencing severe and complex mental illness to ensure a person centred and recovery orientated approach to care planning and coordination.

Additional responsibilities will include:

  • Coordinating with the GP to develop a GP Management plan including promoting the assessment of physical health in people experiencing severe and complex mental illness.
  • Building a therapeutic recovery orientated relationship with consumers.
  • Measuring treatment outcomes utilising validated tools including K10+ and CANSAS P.
  • Assisting consumers with treatment support and medications management.
  • Supporting the development and review of consumers GP mental health care plans and digital health records, to enhance communication links between multi agency service providers.

We have a full-time role available.

The role will target the South Metro region of Perth, however when required you will also travel across the North metro area.

No Shift work, nightshifts, or weekend work.

About you

You will possess the below key attributes:

  • A relevant AHPRA Registration and tertiary qualification or other mental health specific qualifications relevant to the role.
  • Extensive clinical experience working in mental health
  • Demonstrated understanding of supporting individuals living with severe and complex mental illness in the community.
  • Proven assessment skills and ability to develop and review care plans, based on assessed need.
  • Ability to work autonomously, and in partnership with General Practices is a requirement.
  • Experience applying recovery principles when supporting consumers.
  • Ability to coordinate care of consumers and link them with other service providers.
