Your Role

In this rewarding role you will be coordinating and delivering high quality nursing care to client of Freeman House,  who are experiencing alcohol and other drug dependence and / or mental health issues and are at risk of experiencing homelessness.

Your Responsibility

  • Provide comprehensive clinical healthcare services, including detoxification, for individuals with substance dependence, mental health issues, and/or those experiencing homelessness.
  • Collaborate with Freeman House management for client intake and assessment.
  • Maintain effective, open communication, consultation and liaison with clients, healthcare team members and all other members of the Freeman House team
  • Manage access to various medical clinics and services, such as GP, Podiatry, Optometry, Psychiatry, Metabolic Health, Heart Failure, and Smoking Cessation Clinics, alongside mental health support and detoxification.
  • Coordinate and develop health services in alignment with Freeman House management, including contributing to the development and evaluation of operational protocols and procedures.
  • Facilitate communication between prescribing medical practitioners and pharmacies, overseeing medication administration, and maintaining accurate medication records for residential clients.
  • Provide supervision and mentorship for staff involved in medication assistance.
  • Provide support to clients with challenging behaviors when necessary.
  • Deliver holistic nursing care adhering to the Australian Nursing and Midwifery Council's National Competency Standards, while documenting client care accurately and managing electronic health records.

You Will Need

  • Registered General Nurse with a minimum of 3 years clinical experience in a generalist setting.
  • Current registration as a Registered Nurse with the Australian Health Practitioner Regulation Agency (AHPRA).
  • Capacity to communicate effectively both verbally and in writing with clients, staff, management and external agencies.
  • Interest in and understanding of issues relating to homelessness and substance dependence/mental health.
  • Demonstrated ability to work autonomously and as part of a multi-disciplinary team
  • Comprehensive clinical knowledge, particularly to alcohol and other drug dependency.
  • Understanding of and commitment to continuous quality improvements
  • Interest in and understanding of issues relating to homelessness and alcohol and other drug dependence/mental health.
  • Ability to prioritise and manage workloads according to client needs.
  • Ability to establish and maintain professional and appropriate relationships with residents and/or their families/representatives
  • Demonstrated commitment to maintaining confidentiality.
  • Commitment to ongoing professional development.
  • Demonstrated capacity to work with people from diverse backgrounds.
  • Basic computer skills and proven ability to use health information systems
