Hawkesbury, Nepean & Blue Mountains Partners in Recovery aims to support people with severe and persistent mental illness with complex needs, and their families and carers. Nepean Blue Mountains Primary Health Network, in partnership with three community based not for profit organisations including Parramatta Mission, is working to make a difference in peoples' lives by improving how services are delivered across the Hawkesbury, Nepean and Blue Mountains region

We are recruiting for a suitable candidate to lead a team of Support Facilitators, and to engage with key stakeholders to ensure strong program delivery. The role will also include completion of day to day site management and quality checking of service provision/record keeping. This is a contracted service until 30 June 2019.

Selection Criteria

  • Tertiary qualification in Health Sciences, Social Sciences, or demonstrated industry experience with certificate 4 in Mental Health
  • Meet requirements for registration with AHPRA or professional body such as AASW
  • Minimum 5 years experience working in mental health field with severe and persistent clients
  • Demonstrated ability to manage teams
  • Understanding of the Principles of Recovery Orientated Mental Health Practice
  • Experience in case management and developing client centred care plans
  • Sound knowledge on the functions of the National Disability Insurance Scheme (NDIS) and how individual funding packages will assist consumers living with a psychosocial disability
  • Experience in stakeholder engagement, complaints management and strong negotiation skills
  • Advanced communication skills verbal and written
  • Ability to problem solve
  • Proficiency in computer
  • Current driver's license

Desirable Criteria

  • First Aid Certificate
  • Knowledge of Partners in Recovery's program, and its role in assisting consumers transition into the NDIS.
  • Experience using Penelope software
