HR&SS is seeking an experienced ISP planner/case facilitator or graduate. The successful applicant will have considerable training or planning experience relevant to serving clients, ideally working with people with a range of disabilities and complex needs.

The successful applicant will have experience in advising and guiding clients as they develop their care or plan, and knowledge or relevant experience utilising client relationship database systems.

This position offers 28 fixed hours per week managing a balanced case load. Work times are flexible between 9am – 5pm.

Remuneration and conditions: SCHADS Modern Award, Community Development Worker Class II(b) indexed up to year of experience hourly rates, above-Award conditions apply, salary packaging available.
