We are currently seeking an experienced Case Manager to ensure quality complex care supports are provided throughout the Grampians region for eligible recipients of Commonwealth Home Care Packages.

What's the Job?

The position of Case Manager – Ballarat, Ararat and Grampians region will be responsible for the delivery of quality, person centred supports to eligible recipients who have complex needs as an alternative to residential care.

The role requires extensive travel throughout the Grampians regions and to our Northern (Coburg) region office.

The incumbent will utilise a range of internal and external resources available via annecto’ s Home Care Packages, Planned Support and Direct Care programs to assist individuals to remain active and living in their own homes for longer.

This role will also be key to providing leadership, guidance and support to the team that exclusively service the Grampians region, with a view to growing annecto’ s presence in this area of Victoria.

What we need from you:

  • Previous experience in case management / support coordination in a community setting;
  • Must hold relevant tertiary qualifications in health/social science;
  • Demonstrated ability to work autonomously and remotely with a high degree of initiative;
  • Solid knowledge of and commitment to implementing Consumer Directed Care (CDC) and Person Centred approaches
  • Must hold a current Victorian Driver’s License;
  • Knowledge of local/regional community resources and professional support services in the Grampians regions of Victoria;
  • Preparedness to undertake a high volume of travel around the region
