About Your Side

Your Side provides Total Care for older people, people with disability and their carers. 

Your Side has been providing Aged, Carers and Disability Care for over 20 years. Due to our expansion, we are seeking quality driven and experienced ComPack Case Managers to manage Western Sydney and Nepean/Blue Mountains ComPack referrals.

About the Opportunity

This role presents a unique opportunity to be a part of a client directed and quality driven team of ComPack case managers, working closely with a ComPack Team Leader/Supervisor. 

Your Side is focused on building our ComPack case management based on customer experience and quality of services. Your Side is committed to maintaining reasonable and effective number of ComPacks per Case Manager. 

We are looking for several full time and part time team members who would be able to cover either the Western Sydney region (Mount Druitt, Windsor, Blacktown areas) and the Nepean Blue Mountains region (Lithgow, Blue Mountains areas), or both regions.

About the Service

Working with clients, their families and other stakeholders in the sector, the ComPack Case Managers will provide assessments and identify necessary supports for clients, to ensure their safety and care needs are met at home, and to avoid admission or readmission to hospital.

The ComPacks packages aim to provide immediate case management and community support services to eligible clients and their carers to:

  • Foster wellness, reablement, capacity building and greater independence in the community
  • Reduce avoidable admissions to NSW public hospitals
  • Minimise inappropriate referrals to aged care services

This will be achieved through:

  • Assessment in clients home (face to face)
  • Development of care plan including goals and outcomes
  • Providing information on useful and necessary services
  • Making referrals to ongoing services (formal or informal)
  • Phone reviews fortnightly (or as required)
  • Exit planning and review

About You

This exciting opportunity will see you building and supporting a growing service in the Western Sydney and Nepean/Blue Mountains regions. To do this, you will bring the following skills and attributes to the role;

Essential Selection Criteria

  • Currently or recently working as a Compack Case Manager or Team Leader (or equivalent) with a minimum 2 years’ experience.
  • Certificate or Diploma qualifications in relevant Human Services, Case Management or other fields.
  • Experience in working in a KPI environment.
  • Experience and knowledge with Aged Care and Disability services in the above areas, including primary health/hospital system, and being able to advise clients appropriately.
  • Excellent ability to build relationships and rapport quickly and effectively
  • Demonstrated ability to work in person-centred ways with a focus on wellness and reablement
  • Demonstrated ability to build rapport with people, particularly the frail aged and people with a disability
  • Outstanding interpersonal, verbal and written communication skills
  • Valid driver’s license
  • Consent for a National Police Check and a Working with Children Check
  • Right to work in Australia, unrestricted
  • Able to work from home in an appropriate home office set up

Desirable Selection Criteria

  • Relevant experience or qualifications in Social Work, Nursing, Community Services, Case Management, Aged or Disability Care or equivalent  
  • Experience in managing or supervising a team in a KPI environment
