The aim of the Intensive & Case Management Service (I&CMS) is:
- To provide a multi disciplinary, intensive case management youth outreach service to young people who are considered to be at the highest level of risk in the community, and for whom less specialist services have not / would not be effective in ameliorating the young persons circumstances
- Managing the level of risk and providing a comprehensive, contracted case management service to young people aged 12-17 who are considered to be at medium risk, with medium to high needs, who reside, or have resided in Westcare’s out of home care services and are statutory young people.
The service is provided to the young people on an outreach basis, covering the provision of intensive personal support, assessment and individual service planning, monitoring, crisis intervention and case management.
Terms and Conditions as per the Social, Community, Home Care and Disability Services award 2012.
Bachelor of Social Work or Diploma of Welfare Studies or their equivalent or other tertiary qualifications appropriate to public welfare practice.
The Salvation Army, Westcare is a trauma informed agency, therefore a strong understanding of trauma and attachment theory and its relevant application to working with high risk young people is imperative.
The successful applicant will have a great sense of humour and really like working with young people. You will enjoy being part of a dynamic multidisciplinary team, that includes an Alcohol and Other Drugs worker and Mental Health Worker, and be both creative and innovative. You should also have demonstrated case management experience and enjoy working within a team environment. You should have great communication skills with both young people and other professionals. You will need good working knowledge of Child Protection Services, good administrative and verbal skills and a current driver’s licence.
