The Case Manager is responsible for the provision and coordination of quality care that meets the individual needs of the client and the program standards and guidelines. The program provides packages of care to frail older people who have complex care needs and live in the Local Government Areas of Canterbury, St. George and Sutherland.
Tertiary qualifications in Health, Welfare, Nursing or Social Sciences are essential, as is significant demonstrated experience and knowledge in working with older people diagnosed with dementia.
