The Relief Support Worker supports the Case Management Team through participating and undertaking relevant program duties as required in the 19 bed Men's Hostel and 8 Independent Living Units (ILU's). The role of the Relief Support Worker is to ensure the safety and wellbeing of all clients and staff during and outside of business hours.

Client support:

  • Work in collaboration with the Case Management team to provide support consistent with the Case Management Plan
  • Protect the safety and wellbeing of all residents
  • Be aware of Clients rights, standards and relevant legislation and support the client through grievance procedures
  • Undertake and facilitate carried program duties as required including meals programs

Communication:

  • Ensure shifts are commenced on time and preparation has occurred for receiving and giving hand over information to staff
  • Share relevant client, program, training and other resource information with co-workers and management
  • Ensure the client notes and SAMIS data is entered accurately and portrays clients changing circumstances, risk factors and support needs
  • Communicate with the Manager in regards to any Organisation Policy and Procedures

Service delivery and administration procedures:

  • Ensure work practises are consistent with existing service delivery procedures
  • Remain up to date with changes in internal policies and procedures by reading manuals and information bulletins on a regular basis

Domestic duties:

  • All vacant rooms are cleaned within a reasonable number of hours
  • All client linen is washed and restored in the cupboard
  • All office, communal spaces and bathrooms are clean and safe
  • Other domestic duties as required according to shift and or managements direction
