About the role

We have an exciting opportunity for a Senior Case Manager to join our team on a permanent, full time basis (38 hours per week). The Homecare Program delivers Consumer Directed Care Packages to elderly people living in the community to safely maintain their familiar environment. The Senior Case Manager is responsible for the provision of effective, person centred, client support and socially inclusive and trauma informed service coordination to elderly clients with diverse needs whist providing leadership and supervision to a small team.

About you

Essential to your success will be a Degree level qualification relevant to the delivery of community services. You will have highly developed skills and demonstrated experience in case management and service coordination and proven experience in the establishment and management of models for effective client service. You will have proven ability to provide leadership and supervision to a team in delivering high quality services to clients. You will pride yourself on your excellent interpersonal skills, with a demonstrated capacity to work collaboratively with others and exercise influence in a diversity of contexts.
