Based in the outer south-eastern suburbs of Melbourne and covering an area from Greater Dandenong and Casey to Cardinia, South East Palliative Care (SEPC) provides specialist palliative in-home care of the highest standard to people with a life limiting illness according to our core values of Respect, Innovation, Integrity and Enablement.  

The Occupational Therapist role is responsible for working effectively within the interdisciplinary team, providing home-based assessments, case management and care coordination services that effectively support palliative care patients and their families/carers to achieve their care goals and to live well, in partnership with other service providers as required. 

The successful applicant will be a highly motivated and experienced professional, with a passion for and understanding of the issues faced by individuals and families/carers living with a life-limiting illness in their own homes and communities. You have the ability and desire to work to a consistently high standard, both autonomously as well as within a team setting and will demonstrate values consistent with those of SEPC.
