Build strong relationships with Primary Health providers to help them improve clinical practice, business capacity and patient outcomes.

South Eastern Melbourne PHN (SEMPHN) has the key objectives of:

  • Increasing the efficiency and effectiveness of medical services for patients, particularly those at risk of poor health outcomes, and
  • Improving coordination of care to ensure patients receive the right care in the right place at the right time.

Due to expansion and internal promotions, the Provider Support team has openings for new enthusiastic and knowledgeable people to actively participate in the development, support and evaluation of a wide range of SEMPHN Service initiatives. In this role you will:

  • Act as the SEMPHN’s front line staff; communicating PHN priorities to the primary health care workforce and facilitating change
  • Build strong relationships with General Practices and other providers of primary healthcare to support the facilitation of change in their plans
  • Actively help build the capacity, skills and knowledge of primary health care providers
  • Plan, coordinate, implement and support projects such as Health Literacy, Cancer Screening, Practice Coaching, Health Care Homes and Doctors in Secondary Schools
  • Support the development and implementation of commissioned activities.

Initially located in the heart of multi-cultural and vibrant Dandenong, the team will move to SEMPHN’s Heatherton office in 2018.

The successful candidate will have:

  • Tertiary qualifications in either nursing, health, social sciences and/or extensive relevant experience
  • A good knowledge of contemporary issues for general practices and the primary health sector
  • Proven ability to influence positive change to general practices and primary health care providers at the micro level
  • Project skills in Planning, implementation and monitoring and report writing skills.
