An exciting support and case management opportunity exists to work with Queensland Positive People a peer-based organisation which provides innovative and widely recognised support programs for people living with HIV in Queensland.

The treatment support case management role, with a strong focus on client empowerment, is based in Cairns with outreach travel to other regions including Townsville and Mt Isa and provides a variety of experiences and learning opportunities for staff.

Responsibilities of the role are focused on supporting clients to overcome social determinant barriers impacting HIV self-management, through a client-centred case management approach. A key aspect of supporting the achievement of client goals will be establishing and maintaining effective clinical and community access pathways.
