About the Position

The main focus of the position is to is to ensure that the MiCare current client’s quality of life is enhanced through their relationship with MiCare. Your excellent interpersonal skills will enhance MiCare’s position as a provider of choice particularly for migrant communities. The successful applicant will be required to develop local partnerships and referral pathways – particularly within the clinic and the indigenous communities.

Location: Co-located with medical clinic in Morwell (Gippsland region) 3 days, with the ability to work from home up to 2 days a week.

Hours: 23 - 38 hours per week. This role will commence as part time (3 or 4 days) with the view of full time as client numbers and caseloads increase, and with the addition of new funding.

Classification: This position is classified under the Dutch Care EBA. A vehicle or km reimbursement is included in the role. MiCare is a NFP organisation and so employees can benefit from salary packaging.

Qualifications and Experience

  • Diploma in Community Services (Case Management) and/or relevant experience
  • Tertiary education as health professional ie. Nurse / Allied Health / Community
  • Sound knowledge of home care sector
  • Previous experience in case management
  • Experience with delivery of Home Care Packages and Commonwealth Home Support Programme funding (CHSP)

  • Ability to communicate in languages other than English
  • Excellent customer service
  • Excellent organisational and time management skills
  • Excellent verbal and written communication skills
  • An eye for detail, thoroughness in completing tasks
  • Knowledge and experience working with Indigenous groups
  • Ability to communicate effectively with Management, Care Advisors, Support Workers, Service providers and Clients.
  • Victorian Drivers licence

A police check with no disclosable outcomes is required by MiCare for this position.
