About the role

This is a challenging but rewarding opportunity for you to actively support the individual needs of frail, aged and people with a disability who access Life Without Barriers Community Care Services in Esperance. Working as part of a wider team you will facilitate the delivery of quality services and achieve positive outcomes with clients in a positive and proactive environment.

We have casual contracts available with varied shifts.

Key Responsibilities

  • Provide quality direct support to clients in line with their support needs, the program they are accessing and all support plans including their Care Plan
  • Support clients with in-home services such as personal care, social support, medication prompts, meal preparation, in home respite and transport
  • Promote independent living skills and choices
  • Establish a rapport and collaborate effectively with clients and their carer to achieve positive outcomes
  • Ensure a safe working environment at all times and respect client’s rights to privacy, dignity and confidentiality  

Skills & Experience

  • Certificate III in Aged Care, Home and Community Care, Disability, or similar, or willingness to complete this within the first two (2) years of employment
  • A current unrestricted drivers licence
  • A current First Aid Certificate, or willingness to obtain before commencement
  • The ability to develop positive relationships with the clients and to support their individual care plans as appropriate
  • Good communication skills, including the ability to develop and maintain good working relationships with internal and external stakeholders (including carers, families and advocates)

Successful candidates will be required to clear probity checks including National Criminal History Record Check. A First Aid certificate along with current driver licence must be provided before an offer of employment can be made.
