About the Role 

Provide case coordination support services to clients across National Immigration Support Services, so that they are provided with the choice and flexibility in their care planning and transition program to achieve independent community living. This is a 6 month full time position working from the Salisbury office.

Key Responsibilities

  • Apply culturally appropriate client engagement principles and processes to ensure the voice of the client is heard and reflected in case coordination practice and activities.
  • Conduct needs assessment, develop, implement and review individual Care Plans for clients.
  • Deliver skills building modules to clients through face to face group education sessions.
  • Maintain up to date client files, databases and registers.
  • Ensure that all documentation is completed within designated time frames, is of a high professional standard and meets legislative and organisational requirements.
  • Develop and maintain relationships with family members and significant others to promote positive outcomes for clients.
  • Work collaboratively with internal and external stakeholders to meet the needs of the clients.
  • The flexibility to deliver education programs to clients outside of core business hours and on weekends as required.
  • Support and promote the work of Life Without Barriers, maintaining a positive image of the organisation in accordance with level of position.
  • Comply with all Life Without Barriers policy, code of conduct, procedures and practices, external funding body requirements and legislation.
  • Apply and uphold principles of equity and anti-discrimination in the workplace and adhere to organisational and legislative Health, Safety and Environment requirements.     

Skills & Experience 

  • Degree qualification in Human Services or related discipline and/ or equivalent work experience.
  • Strong case coordination experience in the Human Services sector including conducting needs assessments, developing and implementing Care Plans to assist clients with their independent living capacity.
  • Demonstrated ability to support individuals, families and / or groups from a strengths-based and culturally appropriate perspective.
  • Strong interpersonal skills and the ability to delivery engaging education sessions to client groups.
  • Well-developed written and verbal communication skills.
  • High level organisational and time management skills and the flexibility to meet changing demands.
  • The ability to work autonomously and as a part of a multi-disciplinary team.
  • Current drivers licence. 
  • Prior experience in providing professional support to Refugee and Asylum Seeker people.
  • An understanding of adult learning principle would be desirable 

Successful candidates will be required to clear probity checks including National Criminal History Record Check and Working with Children Check (where relevant to the role).
