What’s the role?

Support individuals in achieving a lifestyle of their choice by providing contemporary OT therapy service that maximises independence and participation. 

  • Provide an Occupational Therapy service within allocated portfolio; to provide clinical expertise within the profession.
  • Independent management of OT caseload.
  • Independent management of small Specialist Support Coordination caseload.
  • Community home visits.  
  • Completion of functional assessments. 
  • Facilitating support workers to implement therapy programmes. 
  • Multidisciplinary working with providers involved. 
  • Prescription of equipment and home modifications. 
  • Comprehensive report writing to support participant’s future NDIS Plan recommendations.
  • Opportunities to work along-side sector colleagues in diverse areas (Justice, CPFS, Office of the Public Advocate, Health, other Not-for-Profit organisations.)

Essential Criteria:

To join our existing dynamic team at Facilitatrix, you will have met the following Criteria:

  1. Occupation Therapist qualification with proven complex case management and leadership experience in human services with exceptional engagement skills;
  2. Demonstrated experience in OT functional assessments across a variety of client groups – preferably with adults and children;
  3. Comprehensive understanding of the NDIS, roll out strategy in WA and support coordination processes;
  4. All applicants must attend an Information Session. The Information Session is part of our recruitment process (details below).
